Effective July 1, 2003 This policy covers floral arrangements for funerals of Duke employees and members of their immediate family and for hospitalized employees. Immediate family members include husband, wife, child, mother, father, sister, and brother. Duke (through the individual department) will continue to pay for floral arrangements in the event of a hospitalization of an employee, death of an employee, or death of an immediate family member. When notified by a department of the death of an employee or of a member of an employee's immediate family, the department will call and request a floral arrangement be sent in the name of Duke University and Health System and the department. A small floral arrangement is sent to an employee who is hospitalized at Durham Regional Hospital, Raleigh Community Hospital or Duke Hospital. Memorial Contributions are no longer available. Frequently Asked QuestionsWhat information must be provided? Hospitalized Employees - In the event an employee is hospitalized, the department designee must provide the employee's name, hospital, and room number. In order for flowers to be sent, the employee must be admitted and in an assigned room. The florists will only deliver flowers to employees who are hospitalized at Durham Regional Hospital, Raleigh Community Hospital or Duke Hospital. A 24-hour minimum hospital stay is required. Download a Hospital Request Form and contact an approved florist. Funeral - In the event of the death of an employee or member of the employee's immediate family, the department designee is required to download and complete the Funeral Request form making sure to provide the name of the decedent, relationship to the employee, and name and address of the funeral home including zip code. Designee must also provide the date and time of the visitation. Contact an approved florist. Missed Funeral/Visitation - In the event the visitation was missed or if there is no service, an arrangement can be delivered to the employee’s local home address or the employee’s Duke address only. Employee’s local address including city, zip code, and phone number must be provided at the time of the request to the approved florist. Faculty/Staff, Bi-Weekly, DTS, Part-Time, and Retirees of Duke (does not include RCH and DRH employees). What is written on the card that accompanies the flowers? Duke University and Health System/Department name. Are flowers sent to retirees or people on Long Term Disability? No, flowers are sent in the event of death or during hospital stays. Can flowers be sent to the home of a sick employee? No. When will the flowers be delivered?
Will the University send more than one arrangement when there are several family members working at Duke? No, only one arrangement will be sent per family. Can we send flowers out of state? Yes, in the event of death, a floral arrangement will be sent to the visitation/funeral. Can flowers be picked up at the florist instead of being delivered?
